Illness/Injury Notification Clearance Form--CNICCYP 1700/31

Children who have been excluded from care because of health or medical reasons may need to have an Illness/Injury Notification Clearance Form on file permitting them to return to care with a doctor's authorization. Not all children who are excluded must see a healthcare provider before returning to care. CYP staff should refer to the program-specific Enrollment and Record Keeping Standards and the OPNAV 1700.9 (series) for guidance on when a child must have this form before returning to the CYP.
