The Navy Child and Youth Program (CYP) Birth to Five Child and Family Profile is designed to help our CYP Professionals get to know your child and family, so that they are best prepared to provide a fulfilling and meaningful experience and to ensure your child's needs are met. The CDH checklist used by CDH staff to determine the completion of the CDH process and assist in determining the suitability of CDH applicants.

The CDH Child File Checklist is used in Child Development Homes with child files to ensure all required documentation is current.

CDC, 24/7 Center, and CDH Programs use the Daily Gram to collect information from parents before dropping the child off in care. The form helps Teachers prepare for the child's needs on that specific day. It is also used to share information with the parents about their child's time in care.

Navy CYP Emergency Action Plan

This Emergency Response Report template has been provided as an optional tool for Family Child Care Providers. Facility-based programs are required to post the Emergency Response Report generated from CYMS.

Assists Directors in determining when to open additional classrooms and how to calculate the costs associated with the increased enrollment. The worksheet calculates the additional expenses and show the Non-Appropriated Fund (NAF) profit or loss associated with the projected increased enrollment.
