The Supervision and Accountability Standard provides CYP Professionals with specific guidance regarding the Navy's requirements for facilitating appropriate supervision and accountability measures.

Programs complete the Verbal Request for Release of Child/Youth Form when a parent wants to authorize someone not listed as an emergency contact/release designee to pick up their child/youth on a specific date. For same day requests, parents may complete the form at dropoff or call the program. All phone requests must be verified. This form is only valid for the day noted on the form.

The We Need Your Help Form is used to invite parents to engage with the CYP in collaboratively working toward resolving a concerning behavior. The invitation to collaborate is requested when the CYP has implemented strategies to support the child but resolution is not complete.
