Children who have been excluded from care because of health or medical reasons may need to have an Illness/Injury Notification Clearance Form on file permitting them to return to care with a doctor's authorization.

Immunization documentation requirements have changed for school-age children participating in CYP. Upon the start of the Fall 2021 annual registration/reverification, families with school-age children enrolled in the Child Development Homes, School Age Care, and 24/7 Center programs will be required to provide documentation, or waiver requests, of their child's immunizations appropriate for the child's age per the ACIP to include documentation of the annual influenza vaccine.
