This application is designed to collect total family income data to determine child care fees. CYP professionals use the Navy CYP Parent Fees Calculator to determine Total Family Income (TFI), and enter the data on the DD Form 2652, which is signed by the Sponsor.

The Navy Child and Youth Program (CYP) Birth to Five Child and Family Profile is designed to help our CYP Professionals get to know your child and family, so that they are best prepared to provide a fulfilling and meaningful experience and to ensure your child's needs are met. Please complete the sections below as fully as possible.

The Council of Professional Recognition has developed the Child Development Associate and Military School Age credentials to improve the quality of child and school-age care programs. CNIC CYP will continue to provide supplemental funding to offer CYP employees and CDH Providers an opportunity to continue their training and receive recognition from a national organization. CNIC (N926) has established a centralized contract with the Council of Professional Recognition to streamline the process for funding the CDA and MSA credentials. Enclosures (1) through (4) provide the processes for participating in this opportunity.

The CDH checklist used by CDH staff to determine the completion of the CDH process and assist in determining the suitability of CDH applicants.

The CDH Child File Checklist is used in Child Development Homes with child files to ensure all required documentation is current.

Programs use the CYP Family Rewards Program: Event Points Form to document points earned by CYP parent participants in flat rate activities as a part of the CYP Family Rewards Program.

CDC, 24/7 Center, and CDH Programs use the Daily Gram to collect information from parents before dropping the child off in care. The form helps Teachers prepare for the child's needs on that specific day. It is also used to share information with the parents about their child's time in care.
