The Medication Authorization Form is required for all children who need to take prescription medications while at the CYP.

The Medication Dispensation Log provides a written record of the medication being given by CYP staff to the child (when it was administered, by whom, and the dosage given).

The form is used to record information that is used by the CYP Personnel to report incidences of Accidents, Incidents, Child Abuse and/or Neglect of the CYP. Disclosure of requested information is voluntary.
