Families use this form when they are enrolling and registering their children to document in writing their releases and authorizations for field trips, transportation, topical non-prescription product application, media release, an acknowledgement that they have received the Navy CYP Parent Handbook, and a general hold harmless release.
During the parent orientation prior to each program, the Navy YSF Parent Fact Sheet will be issued and discussed with the parents. The information within the Parent Fact Sheet concerns the basic rules of the sport/activity, equipment and uniform requirements, safety issues, promotion of good sportsmanship for both children and parents, skill development for their child, and information on how to complete the Parent Association for Youth Sports (PAYS) training.
The Program Accommodations Form (Inclusion Support Information and Referral) documents the program accommodations required to support individual needs, whether accommodations can be provided within existing program operations and with existing resources, and the program steps taken to support the child.
CYP professionals use the Navy CYP Parent Fees Calculator to determine Total Family Income (TFI) and enter the data on the DD Form 2652, which is signed by the Sponsor.
All families registering with Navy CYP must complete a Parent Fee Agreement for their participating children. The Fee Agreement must be completed electronically by a CYP employee and signed by the sponsor. Agreements must be updated annually.
