Follow the steps below to get the forms and documents you need to complete your registration and enrollment.

24/7 Center Enrollment Steps

    Download the forms and materials you need.

    • Download these forms onto your computer. (Click on the form and when it opens, save the form/document to your computer. When you fill it out, save it again.)
      • Some forms are fillable. If a form is fillable, complete it and print it with all other documents.
      • If you need help downloading the forms or filling them out, please call your local program. The contact and number is listed in your email.
      • You may also choose to complete your forms at the program office, your CYP Professionals will be happy to help you.
    • If your confirmation email mentions a deposit, please make sure you pay within 48 hours. Your child's space is not guaranteed until you have paid your deposit. Refer to your email for details

    Regularly Enrolled Children from Birth to the Start of Kindergarten

    Next, email the completed forms to us.

    Once you have completed all forms, save them to your computer and then send all the forms to the email address provided in your enrollment email.

    Please email these forms no later than 24 hours before your scheduled family orientation.

    When you come to the CYP, bring the following:

    • Proof of eligibility (e.g., military ID card)
    • Your monthly (or longer) work schedule signed by your command (if shift worker/watch stander)
    • Your child's immunization records (for children of any age who have not yet started school, or who have been homeschooled and are not enrolled in any other CYP offering)
    • Income verification documents (read The Family's Guide to Total Family Income sheet).
    • Notify the 24/7 Center of an identified special need prior to orientation.
      • Bring necessary documentation, e.g. Individualized Education Program, Emergency Action Plan, to the program.
    • You may return these forms via email in advance.
