CYPs complete this form to inform families of upcoming off-base field trips or special events for their children/youth. Forms must be created for each event. Families sign and return completed forms to give permission for their children/youth to participate. Families may use one permission slip for all participating children/youth attending the event.

This Food Substitution List template has been provided as an optional tool for Family Child Care Providers. Facility-based programs are required to post the Food Substitution List generated from CYMS.
