The Navy Child and Youth Program (CYP) Birth to Five Child and Family Profile is designed to help our CYP Professionals get to know your child and family, so that they are best prepared to provide a fulfilling and meaningful experience and to ensure your child's needs are met. Please complete the sections below as fully as possible.

The Navy Child and Youth Program (CYP) Youth and Family Profile is designed to help CYP Professionals get to know the children, and youth enrolled in our School Age Care programs. The information gathered will be used by CYP professionals to develop relationships and activities to better serve our customers.

The Navy CYP Registration Form is used by families to enroll children and youth in a Child and Youth Program.

The Medication Dispensation Log provides a written record of the medication being given by CYP staff to the child (when it was administered, by whom, and the dosage given).

The Program Accommodations Form (Inclusion Support Information and Referral) documents the program accommodations required to support individual needs, whether accommodations can be provided within existing program operations and with existing resources, and the program steps taken to support the child.

This quick reference guide provides instructions for individuals serving the Exceptional Family Member Program (EFMP) to navigate the enrollment and Family Travel Screening processes for the Army, Marine Corps, Navy, and Air Force. An introduction to EFMP Family Support is also included for your reference.

A GPAB tip sheet has been developed to assist in clarifying some business practices around executing the policy at your installation.

Active duty members are often separated from spouses, as well as extended family members who might otherwise offer support. In an effort to assist families, supplemental funding has been provided to offer respite care through the CYP "Give Parents A Break" program.

The policy clarifies information to installations on requirements of references: BUMEDINST 6230.15A and OPNAV 1700.9E in their bargaining agreements regarding immunization requirements for both employees and children in Navy CYP.
