The Navy CYP Registration Form is used by families to enroll children and youth in a Child and Youth Program.

The Self-Release Form allows parents, based on the Installation's home alone policy, to authorize self-release privileges for their eligible youth.

Parents complete the Youth Sports and Fitness (YSF) Supplemental Information Form after signing up their youth in seasonal sports leagues to indicate the sport, their youth's uniform size, and interest in volunteering.

The form is used for an employee to request an exemption to an immunization.

The form is used for the parent to request an exemption to an immunization for their child.

The form is used for a CDH Provider to request and exemption to an immunization.

The Family Communication Tracking Log is used to document all communication attempts--by phone, email, cubby memo, or in-person--with a family when requesting record updates for immunizations, flu vaccinations, employment and/or student status documentation, and/or other required documents.

Families use this form when they are enrolling and registering their children to document in writing their releases and authorizations for field trips, transportation, topical non-prescription product application, media release, an acknowledgement that they have received the Navy CYP Parent Handbook, and a general hold harmless release.

Families use this form when they are enrolling and registering their child to let the program know of any identified needs their child may have, including food or other allergies, medication needs, food intolerances and dietary needs, and any early intervention or special education needs.

Effective immediately, all CNIC FFR System Access Requests, to include CYMS, Kronos, FFR SAP Portal, SAP ERP, SAP HR, FFR Navy Single Sign On, JIRA and Privileged User Agreements will now be using the CNIC FFR System Access Request Form version 1.1/January 2019.
