The Navy Child and Youth Program (CYP) Birth to Five Child and Family Profile is designed to help our CYP Professionals get to know your child and family, so that they are best prepared to provide a fulfilling and meaningful experience and to ensure your child's needs are met. Please complete the sections below as fully as possible.

The Medication Dispensation Log provides a written record of the medication being given by CYP staff to the child (when it was administered, by whom, and the dosage given).

This booklet will give you an overview of the laws pertaining to inclusion and how they relate to your child care or youth program. You can use this information to ensure your program compiles with federal laws protecting children and youth with disabilities.

The Navy Child and Youth Programs Interactions and Relationships Standard provides CYP Professionals with specific information and guidance regarding the Navy's requirements for facilitating appropriate and healthy interactions.

The CYP Guidance and Touch Policy is designed to inform CYP Professionals of the boundaries for appropriate and inappropriate adult:child touch to create a safe, healthy, and positive environment for children and youth.

Families use this form when they are enrolling and registering their child to let the program know of any identified needs their child may have, including food or other allergies, medication needs, food intolerances and dietary needs, and any early intervention or special education needs.

Navy Child and Youth Programs are required to establish administrative procedures, record keeping and file maintenance systems to ensure information is organized, accurate, and readily accessible. The Navy CYP Enrollment and Record Keeping Standards provide CYP professionals with specific information and guidance regarding requirements for record keeping and file maintenance for enrolled children and CYP professionals.

We all play a part in making sure the youth in our programs feel included and safe by ensuring that the physical environment, activities, and programs offered are welcoming to all youth. This guide provides youth program managers and training specialists with strategies for implementing practices within their programs to promote a sense of safety, belonging, and purpose for all youth.
