The Vacation Notice Form must be completed by parents to notify the CYP of their intent to take vacation.

Programs complete the Verbal Request for Release of Child/Youth Form when a parent wants to authorize someone not listed as an emergency contact/release designee to pick up their child/youth on a specific date. For same day requests, parents may complete the form at dropoff or call the program. All phone requests must be verified. This form is only valid for the day noted on the form.

This form is used to request funding for purchasing vehicles for Child and Youth Programs.
