CYPs complete this form to inform families of upcoming off-base field trips or special events for their children/youth. Forms must be created for each event. Families sign and return completed forms to give permission for their children/youth to participate. Families may use one permission slip for all participating children/youth attending the event.
The FCC Office Comprehensive Inspection is to be used during the annual comprehensive inspection for the Installation. CDH Programs not assigned to a specific Installation (i.e. Metro or Regional CDH Programs), must coordinate with Preventive Medicine to ensure the CDH Office receives a Comprehensive Inspection annually.
