CYPs complete this form to inform families of upcoming off-base field trips or special events for their children/youth. Forms must be created for each event. Families sign and return completed forms to give permission for their children/youth to participate. Families may use one permission slip for all participating children/youth attending the event.

The Family Relations Standards provide operational requirements with guidance on, and strategies for, facilitating implementation of all Navy policy requirements for building and sustaining collaborative relationships with families.
