The Navy Child and Youth Program (CYP) Birth to Five Child and Family Profile is designed to help our CYP Professionals get to know your child and family, so that they are best prepared to provide a fulfilling and meaningful experience and to ensure your child's needs are met. Please complete the sections below as fully as possible.

The Navy CYP Registration Form is used by families to enroll children and youth in a Child and Youth Program.

The CDH checklist used by CDH staff to determine the completion of the CDH process and assist in determining the suitability of CDH applicants.

The FCC Health and Sanitation Checklist provides Child and Youth Programs (CYP) with information used to access the FCC provider's program and to ensure that the program is meeting minimum Navy FCC requirements. Identifies areas for future training and monitoring.

In order to support mission requirements, Commander, Navy Installation Command (N926) has established standard subsidies, which are based on geographic location, to offset the cost of child care to families.

To provide Child and Youth Programs (CYP) with information to determine the qualifications, suitability and availability of applicants for volunteer positions within the CYP.

A poster reflecting these steps is required to be posted at each diapering location.
