Recreational Program Enrollment

Follow the steps below to get the forms and documents you need to complete your registration. We need ALL these forms.

    Download the forms and materials you need.

    • Download these forms onto your computer. (Click on the form and when it opens, save the form/document to your computer. When you fill it out, save it again.)
      • Some forms are fillable. If a form is fillable, complete it and print it with all other documents.
      • If you need help downloading the forms or filling them out, please call your local program. The contact and number is listed in your email.
      • You may also choose to complete your forms at the program office, your CYP Professionals will be happy to help you.
    • Go to the YP office to complete your registration.

    When you come to the office, bring the following:

    • Proof of eligibility (e.g., military ID card)
    • Your youth's immunization records (for youth of any age who are homeschooled, and are not enrolled in any other CYP offering)
    • Notify the YP of an identified special need prior to orientation.
      • Bring necessary documentation, e.g. Individualized Education Program, Emergency Action Plan, to the program.
    • You may return these forms via email in advance.

For Youth Sports Only

  • NAYS Parent Orientation  (not required for Start Smart)

This is part of your registration requirement. At least one parent must complete NAYS Parent Orientation before their youth can participate. Please follow the instructions in your registration email for instructions on how to complete the training and the deadline by which you need to complete it. Contact your program office if you have any questions.
